For routine wellness visits such as vaccines, rechecks and routine lab work, please call us at (863) 293-0850 or you can fill out the form below. Please note that this is to REQUEST an appointment only, and appointments are not finalized until approved by a member of our team.

PLEASE NOTE: We see appointments between 9:00 am - 12:00 pm and 2:00-4:30 pm, on the half hour. On Saturdays, we see appointments between 9:00 am - 11:30 am, on the half hour. Please request an appointment during these times.
